NHS reforms in the Western Isles


The issue of service reforms in the NHS in the Western Isles has dominated the news over the past few weeks. Without boring readers to tears, there are a few issues that influence the NHS in the Western Isles. First of all, only 26,500 people live between Ness and Mingulay. There has to be a fully equipped health service on the islands, in order to cope with any immediate emergency. We have a largish hospital in Stornoway, a medium sized one on Benbecula and a small unit in Barra.
It is common practice for complicated cases to be flown out to major centres like Inverness and Glasgow. Health professionals, such as doctors, have to maintain and expand their skills, and to that end usually rotate through different hospitals over a period of years. Because complicated cases are not normally treated at Stornoway (or Benbecula or Barra), there is little professional incentive for doctors to come here. In other words, it is desperately difficult to attract professional staff. The same applies to other health professionals, like nurses.

The Health Board has therefore been forced to get the necessary staff in as locums. These can cost as much as 拢70 an hour, and if they are on call (i.e. available to work, but not necessarily on the job), that can mount up.
At the moment, there is a vacancy for a psychiatrist after the previous consultant retired on December 6th. He announced his retirement in July, but the Board never took action until much later than that. The result is that no-one has as yet been found to replace the consultant in question, and another locum is probably going to be employed. At a considerable cost.

So, the NHS Board in the Western Isles decided on service reform to address this problem. In consultation with staff, the Board reached the conclusion that closing a ward, to release nurses for duties elsewhere in the NHS (not necessarily in hospital), would be a good idea. And that skills and responsibilities would be shared out to lower grades of professional staff than before. Training would be provided where necessary. An example: a general surgeon can be expected to perform a caesarian section. Psychiatric emergencies are expected to be dealt with (out of hours) by a community psychiatric nurse and a GP. Still with me?

Second problem is a serious breakdown in communication between management and staff. Allegations of bullying and harassment have been flying around, and are currently being investigated. People felt so intimidated and brow-beaten that they did not feel able to speak to their manager about any concerns or misgivings regarding the current round of reorganisations. They felt able to speak to anyone, apart from the management of NHS Western Isles. As a result, local councillors organised a meeting on November 30 for staff to air their grievances. And air they did. It was a damning indictment of the Health Board, which has been widely reported in the press. The Health Board itself put its case to the public the next day. When the opportunity arose for questions, several people voiced their concerns over service cuts and the problem of staff morale. The Chief Executive himself did not acknowledge or respond to the criticism - he mutely passed it on to one of his medical directors to answer. It was this same Medical Director who has gone on record today (16th December) as saying that any more criticism of the health board may well lead to its abolition. In other words, criticism is not allowed. That is also bullying. The Scottish Health Minister, Andy Kerr, has said that he is not interested in the bullying allegations. Whilst these in themselves may justly be seen as a purely internal matter, they very seriously undermine the credibility of the health board when it states that the proposed service changes were made in proper consultation with staff. It is speaking volumes that grievances can only be aired in public meetings, at the instigation of local councillors. Under normal circumstances, this should be thrashed out internally without the need to hang out all the dirty washing.

The third problem that surfaced were the facilities offered to Health Board managers not to have to live in the islands, but being able to commute from and to the mainland at the tax payers' expense. They were held not to contribute to the island's economy in a direct sense. Unfortunately, it would appear that (unlike e.g. elected representatives, such as councillors) Health Board managers are appointed, and are not accountable to the public they serve.

The hospital in Stornoway has always been 'of the people', built with the money of the people of Lewis and Harris. People are very proud of their hospital, and feel strongly about cutbacks in services.

To me, as onlooker, this smacks of an old attitude that used to be around in these islands. It reminds me of "A Shilling for your Scowl" i.e. do not criticise the man in the Big House - Laird knows best. It is an attitude I've never understood, yet it does keep rearing its ugly head.

I thought we had gone into the 21st century, not still stuck in the 19th.

We struggle to get professional to the island too although the main problem is the high house prices and cost of living combined with the end of progression to careers. The High School is always struggling to attact teachers, although if the existing teachers who live in school houses who own houses which they rent out at huge rents just lived in the properties they own on the island or sold the properties they own on the mainland and used the money to buy houses here they would free up the school houses for young teachers who want to move here. We've been trying to attract a dentist for over 5 years now. Again house prices and the difficulty to advance ones career seems to put people off even though the post has been upgraded to offer a better salary. Most of us have to go to the mainland for a check up. I haven't had a check up in over a year because it's so difficult to organise so far in advance.

It might be worth looking back at the Advert which was used for the recruitment of the present Chief Executive, as a point of refrence as it was stated that they should be located on the Island...which begs a question ...more over it requires an answer from those who are allowing this pratice to continue. It is also worth noting that not only do they not contribute to the economy and life of the islands, they and thier families do not depend on the very services they are "redesigning" for us that do. It is no wonder that staff are reportedly 'worried about speaking out'. if such a vieled threat issued by medical director quoted above, is made in public what must it be like inside the organisation?

It is disapointing to see yet more revalations in the press regarding what can only be called a finacial debacle within 'Our Local NHS' it looks as this it will overshadow all the other reported 'people' issues like bullying, harasment and threatning behaviours in and outside the organisation. But where does that leave the 'Services' the staff that provide them and those that need them , (unless of course you live on the mainland). we all depend on these services. If as reported closing a ward will save 拢750K and there is a 拢3 Million + overspend. Then should we be asking what does the full amount equal ...a whole hospital perhaps? ? Bearing in mind that ALL public Bodies spend heavily in the last quarter. Could NHS WI be looking at a figure greater than 拢4 Million maybe 拢5 million +, which I am led to belive would be near 10% of the total budget. Is it really possible that 'Good Houskeeping measures' will, as reported today, really ensure that this will be enough to get this spiraling debt under control, NOT involve redundancey, NOT mean more closures or worse...What has gone so wrong in the last few years??

i think that it should be noted that prior to the installation of the existing executive board that the reported deficit for the WIHB was around 拢230, 000.00. What could possibly have brought the present reported overspend to its new total of over 拢2 million. Commuting would certainly not help. The director of finance should also be held accountable as housekeeping obviously seems to be a problem for this individual. Department managers are being asked to pull their belts in and if they are unable to keep to the budgets they are held accountable so this should also apply to executive members who are supposedly well educated and trained in the business world to run an organisation effectively. I am under the impression that if you read between the lines that these people may have been put in place by the Scottish Executive deliberately to undermine the WIHB and bring about its demise as a Board and therfore ensure that it becomes swallowed up by a trust. There seems to be no other explanation why the Scottish Executive is ignoring the pleas of the local people for help on this issue. The recent statement by Alasdair Morrison Labour MSP that this should be dealt with locally is ridiculous. It has long since gone past that stage. The local population of Stornoway have the right to expect Mr Morrisons fullest comittment on investigating these allegations and taking our concerns to the highest level of parleiment. It is felt by many people to whom i have spoken that he seems reluctant to act on our behalf...he has given assurances that the board will continue to bea board but then again many politicians have said what they think you want to hear at the time while all the time being aware of the actual agenda, to indicate a possible hidden agenda would be a bit risky incase it interferred with the process at Scottish Executive level, he wouldn't be the first to do this sort of thing and won't be the last. It would suit him well to remember that he did not just happen to wake up one morning to find that his position was granted to him freely....he was voted in by a majority of the population and that the islands population pay enough in taxes that ultimately contributes to his salary. Would he have something different to say if a member of his family were to be discharged from hospital prematurely. As for the current Medical Director....well his comments leave alot to be desired. For a man of his professional capacity he should be more careful about his public comments.....this has on ocassion been detremental to the boards position...his comments at times have been considered by members of the public as dangerous and could be considered as bullying. He has been discussed by members of the public as being a liability to the WIHB. There was a day that this islands population would have come out in force and stood up to this sort of treatment ......but intimidation is such that fear of reprecussions are holding the employees back...the WIHB is as you know the second largest employer on the island and jobs are few...this i think is another gun that is being held to the heads of the WIHB employees.....bullying, intimidation and staff morale does exist in th WIHB and you only have to walk round and listen to the whispers to know that it is indeed a problem. The island has a small population in comparison to other areas and a hospital of this kind does not need endless tiers of management , which i might add has incresed quite dramatically in the past year. get rid of these layers and put more staff on the floor where its needed. staffing levels on ward levels is often desperate but due to budget restrictions much needed nursing staff can not be placed. The staff in the Western Isles Hospital should be appluaded for their comittment to the services they provide and they should be allowed to continue to work without fear or intimidation.I wish all the staff the best wishes with their endeavours to have executive management removed.
