10 years ago, on 15 November 2012, Matthew Leahy died at an Essex mental health facility.

It's been a long journey for Matthew's family, particularly his mother Melanie who has been campaigning for answers about how he died.

Concerns have been raised about patient safety at EPUT - Essex Partnership University NHS Foundation Trust. It has already been fined 拢1.5 million for safety failings relating to the deaths of 11 patients in its care (from March 2022).

Paul Scott, Chief Executive of Essex Partnership University NHS Foundation Trust (EPUT), said in a statement:
