The Western Trust has been heavily criticised in a report exposing failings involving a child in its care. Vicky - which isn't her real name - has been in the care system for most of her life and is now in a medium secure unit in England. A formal investigation by the Northern Ireland Children's Commissioner found a catalogue of failings over 21 years.
