On 23 November, Ethiopia鈥檚 Fana Television broadcast a 50 second-long segment of Prime Minister Abiy Ahmed speaking in what appeared to be the national parliament. 成人快手 Monitoring reported that the PM had said the Tigray operation would continue 鈥渘o matter who dies鈥, based on a comment in the recording. We later established that the clips were not current and did not refer to the Tigray situation.
We have since deleted the tweet.

In exchanges about the Test and Trace system a panellist said that 240,000 tests were being done a day. We suggested this referred to the capacity for daily testing that exists, not the actual number of people who were being tested, which we said was 81,000. In fact 81,000 was the average daily number of people newly tested in England and did not include those newly tested in other parts of the UK.

We reported that the National Audit Office had found a potential conflict of interest in the awarding of a Covid 19 contract to a firm whose founders previously worked for or advised the Cabinet Office minister, Michael Gove.
The NAO found 鈥渘o documentation on the consideration of conflicts of interest, no recorded process for choosing the supplier, and no specific justification for using emergency procurement.鈥
But we should be clear that it also found 鈥渘o evidence that the Minister for the Cabinet Office had been involved in either the award or management of this contract.鈥

In the episode of Thinking Allowed which was first broadcast on 11 November 2020, the presenter gave his opinion about the Head of the UK Vaccine Task Force, Kate Bingham, querying whether recent allegations that Ms Bingham had disclosed confidential information at a US conference might be considered an example of contemporary corruption.
Ms Bingham was not given an opportunity to address this prior to broadcast. Ms Bingham has told us that she did not disclose confidential information at the conference and that she gave the presentation in her capacity as Head of the Task Force, which is a role she performs unpaid for the public benefit. The content of her presentation received prior approval from officials which the government has confirmed.
We apologise to Ms Bingham for not giving her the opportunity to respond prior to broadcast.

In exchanges about the death rate from Coronavirus in different parts of the UK we said that it was 26 per 100,000 people in Northern Ireland as compared with 46 per 100,000 in England and Wales, and 51 in Scotland.
While data from the four nations are not directly comparable as methodologies and inclusion criteria vary, we should have used more up to date figures. The latest Government figures for 30 May show that in fact the death rate from Covid-19 in Northern Ireland was 28 per 100,000 as compared with 58 per 100,000 in England and Wales, and 44 per 100,000 in Scotland.

ASOS has asked us to clarify that the comments are inaccurate and has provided the following statement: 鈥淎SOS did not cancel billions of pounds worth of orders in Bangladesh, and did not cause any deaths of any workers in the country as a result. ASOS does not strategically destroy garments returned by customers to avoid reprocessing costs. 97% of the returns ASOS receives are resold after going through an inspection and, if necessary and possible, a cleaning and repair process. For the remaining products ASOS works with third parties to ensure that the garments are repurposed, resold, or recycled where possible.鈥
The version of the programme which is currently available has been edited to remove the comments. We apologise to ASOS.

On 8 October, the 成人快手 Arabic Trending programme broadcast an item on Ahlam Al-Tamimi鈥檚 appeal to the King of Jordan to be re-united with her husband.
Ahlam Al-Tamimi is a Jordanian citizen who was convicted in Israel and sentenced to multiple life sentences after 15 people including 8 children were killed in a suicide bombing in Israel in 2001. She was released to Jordan in a prisoner swap between Hamas and Israel in 2011 but remains on the FBI鈥檚 most wanted terrorist list for conspiring to kill US nationals.
This segment of the programme was in breach of our editorial guidelines and we have removed the clip from all our digital platforms. It should not have been shown and we apologise for the offence caused.

We stated that under EU law refugees are required to claim asylum in the first safe country in which they arrive. In fact, the Dublin III regulations to which we were referring state that a refugee should normally claim asylum in the first EU country in which they arrive, and in some circumstances can be returned there should they subsequently travel to another EU country. There is however no legal requirement for them to do so.

成人快手 South Today and subsequent 成人快手 news outlets reported on a bus crash in Winchester in which footage was shown of the immediate aftermath of the crash, inside the bus, taken by a pupil.
The video was used in two programmes run by the channel - 10pm news on Thursday 10 September and the lunchtime news programme on Friday 11 September. The video was also uploaded onto the 成人快手 South website and shown on 成人快手 South Today.
Prior to showing the video, a distress warning was given, but in retrospect we believe we were wrong to show the footage before the nature and seriousness of the injuries were known, and with regard to the issues of infringement of privacy. We are sorry about any distress that we caused to the families and the children involved.

In fact Human Rights Watch says that across Brazil police have killed 33,000 people in the last ten years. This includes close to 9,000 killed by the police in Rio and Human Rights Watch says three quarters of these deaths were black men.

We reported on the problems experienced by one homeowner, Gavin Ward, who had cavity wall insulation installed in his property, guaranteed by the Cavity Insulation Guarantee Association (CIGA), in 2011. The guarantee provides assurance that defects with workmanship or materials in connection with the installation will be rectified free of charge for 25 years up to a limit of 拢25,000.
We have been asked by CIGA to make clear that it became involved late last year after Mr Ward first turned to the company that installed the insulation, Miller Pattison, via a claims company, in 2017, for damages to cover the cost of fixing his house.
Mr Ward鈥檚 surveyor found the cavity wall insulation (CWI) had failed and put the cost of extracting it and carrying out repairs at 拢60,000. Miller Pattison disputed the claim after its own surveyor said dampness in the property was not related to the cavity wall insulation and no remedial work was necessary.
Mr Ward moved out of the property in 2018 after it was deemed uninhabitable.
Miller Pattison went into administration in 2019 before the two sides were able to reach an agreement.
CIGA accepted that CWI should not have been installed and has offered to fully extract it. However, because Mr Ward had not notified CIGA until two and a half years after he first became aware of the problems in his property, CIGA says it is unable to assess the interior damage which resulted from the faulty cavity wall insulation rather than damage that has occurred in the meantime and for which it cannot be held responsible. As a result it cannot consider any claims for internal works.
CIGA says Mr Ward can challenge its decision by taking his case to arbitration.

He was actually quoted by the Ethiopian News Agency as saying: "We are tired of begging and the desire to develop does not mean we have intentions to harm other countries".
We would like to apologise for the error, which was due to a misinterpretation of these comments and which has since been corrected on the page and in corresponding social media posts.

In a review of the newspapers we referred to a Daily Telegraph story in which the former head of MI6, Richard Dearlove, said he believed the Coronavirus pandemic started 鈥渁s an accident鈥 when the virus somehow escaped from a laboratory in China. Our review said he'd seen new research by a Norwegian-British team, which claims to have discovered clues in COVID 19's genetic sequencing, suggesting it may not have evolved naturally. We also quoted the Daily Telegraph as saying "the study has not yet been accepted for publication in any scientific journal." In fact the Daily Telegraph reported that research was "judged to be of sufficient scientific merit to be accepted for publication in the Quarterly Review of Biophysics Discovery, a journal chaired by leading scientists from Stanford University and the University of Dundee." The Daily Telegraph went on to report that it is further analysis by the same team on this subject which has not yet been published.

We would like to make absolutely clear that Emily Maitlis was not 鈥榬emoved鈥 or 鈥榮uspended鈥 from last night鈥檚 programme, despite much speculation to the contrary. She herself has tweeted that she 鈥榓sked for the night off鈥.
The 成人快手 must uphold the highest standards of due impartiality in its news output. We reviewed the entirety of Newsnight on Tuesday May 26th, including the opening section, and while we believe the programme contained fair, reasonable and rigorous journalism, we feel that we should have done more to make clear the introduction was a summary of the questions we would examine, with all the accompanying evidence, in the rest of the programme. As it was, we believe the introduction we broadcast did not meet our standards of due impartiality. Our staff have been reminded of the guidelines.
Newsnight has a long-established and recognised reputation for excellent journalism, for scrutinising arguments and for holding power to account, which it does on a daily basis, including the night in question.
Our editorial guidelines allow us to make professional judgments but not to express opinion.
The dividing line can be fine, but we aim to say so if we think we have overstepped the mark.
The introduction to Newsnight was intended as a summary of the issues that would be explored, with all the supporting facts and evidence, in the programme. But as broadcast, it risked giving the perception that the 成人快手 was taking sides, and expressing an opinion, rather than being impartial.
It said that 'the country' was 'shocked the government cannot see' Dominic Cummings broke lockdown rules; that he 'made those who struggled to keep the rules feel like fools'.
But there are some who do not share this opinion, nor think that the issue is a 'scandal' or the Prime Minister has displayed 'blind loyalty'.
By presenting a matter of public and political debate as if the country was unanimous in its view, we consider Newsnight risked giving the perception that the 成人快手 was taking sides - or that the introduction constituted the presenter's opinions, rather than a summary of the journalism which would follow, which explored these issues rigorously and fairly and, crucially, with the supporting evidence.
This is not a question of apportioning blame to anyone.
It is a question of accountability to our audiences.
Our audiences hold the 成人快手 in high trust, not least because we hold ourselves to exacting standards, and we do not want to forfeit this by ignoring our own rules.

While the regulations now differ for people living just a few miles apart, on either side of the border, we have been asked to make clear that the policing of travel restrictions in Wales is continuing.

We received complaints from some viewers about the background information we provided about contributors to this edition of Panorama that dealt with, among other things, the inadequacy of the personal protective equipment (PPE) being provided to frontline NHS staff.
The interviewees in question were all selected on the basis of their direct NHS experience and expertise.
We spoke to dozens of health care workers during the making of the programme and they all had concerns about the lack of proper PPE. These concerns have also been reflected by the British Medical Association and the Royal Colleges.
None of our interviewees was sourced through a political party, union or professional body. Each was speaking from their own personal experience of providing care to patients with COVID-19. One of our interviewees was a trade union representative and was identified as such in the programme.
One contributor, Dr Sonia Adesara, stood as a Labour candidate in a local election in 2017, and appeared in a Labour Party election broadcast before last year's election.
We acknowledge that mentioning this would have helped viewers make their own assessment about her comments, although we do not consider it cast doubt on the validity of her concerns: Panorama established for itself the shortages that concerned her, including the rationing of PPE in high risk areas.
The programme's revelations about the PPE shortages did not come from any of the contributors and have not been disputed since the programme was broadcast.

We reported across different outlets and platforms that a boss of an NHS Trust had contacted the 成人快手 with concerns about the shortage of gowns for the Coronavirus crisis. He had asked us for the phone numbers of Burberry and Barbour - both trying to aid supply.
The person concerned is not in fact the boss of an NHS Trust but part of a network of organisations helping to source personal protective equipment for some Trusts.
The mistake was caused by a misunderstanding of the person鈥檚 role in the fight against the pandemic.

The programme included a report about the release of a study from the National Institute of Economic and Social Research (NIESR), which found that by 2029 Boris Johnson's Brexit deal would leave the UK 拢70bn worse off than if it had remained in the EU.
When introducing this report the newsreader misspoke, inaccurately referring to how the NIESR forecast that the deal would "...leave the UK 70 billion pounds a year worse off".
